Follow steps 1 and 2 below to provide information about people involved in your business.
Entity-owned firms do not need to complete step 1.
Your firm's officers, directors, and member representatives must complete step 2 (formerly SBA forms 1010-IND and 912). Step 2 has some questions entities are not required to answer, so follow the supplemental instructions for entity-owned firms to complete these questions.
If you do not own at least 51% of your business, you must have other owners who can claim disadvantage fill out this section.
The spouses of all owners claiming disadvantage must complete the spouse section.
