Request information outside of Annual Review process

This demo will walk a user through requesting information from the firm outside of the annual review process. The walkthrough steps are as follows:

1. Click the on the firm profile to initiate the workflow
2. Complete the steps for the walkthrough
3. The new custom application will appear in the firm profile
4. Open the new custom application, you will see a card with a title set in the workflow

The prototype stops at the overview page for the new custom application. The prototype does not include what appears when the user clicks on the card.
